How do I prepare for my first appointment?

Please arrive 15 minutes in advance to fill out new patient forms. Please bring your health insurance cards, referral form if necessary, copay, full list of all medications including dosage and frequency, name and phone of primary care physician, and recent test results. If you have to cancel please notify the office at least 24 hours in advance.

What insurance plans do you participate with?

We participate with most plans, including Medicare. Please call your insurance company for confirmation or contact the office with any additional questions.

What should I do in case of an emergency?

Call 911 or go directly to the emergency room. The physicians are available 24 hours a day, seven days a week.

If you call after the office is closed the answering service will contact the on-call physician who will then call you back.

How do I get a prescription refill?

Please call the office at 201-568-3690 during regular office hours for prescription refills. Please allow 24 hours for all renewals unless an emergency.
